Based on interview and record review, conducted during a site visit on 02/28/24, it was confirmed the facility failed to document all required elements of an investigation of reports of abuse and suspected abuse. Findings include, but not limited to the following:

A review of the facility' s investigation of abuse or suspected abuse into missing liquid morphine for an unsampled resident was missing the following required elements:

·Time, place and individuals present; and

·Administrator's review

In a phone interview on 03/01/24, Staff 1 (Executive Director) stated the nurses did an investigation into the narcotics and likely gave him/her the most concise version of the investigation.

No other investigation was provided containing all of the required elements.

The facility failed to document all required elements of an investigation of reports of abuse and suspected abuse.

The findings were reviewed with and acknowledged by Staff 1 by phone on 03/01/24.

Verbal plan of correction: Staff 1 has ODHS abuse investigation and reporting guide. Training to be provided to facility LPN and RN and training with staff will be completed on Monday 3/4/24.
