Based on record review, observation and interview it was confirmed the facility failed to provide a call system that connects resident units to the care staff center or staff pagers. Finding include but not limited to;


During an unannounced site visit on 06/09/2022, Compliance Specialists (CS) tested electronic call system in resident rooms and facility bathrooms and observed they were non-functioning.


In separate interviews on 06/09/2022, Staff 1-5 (S1-S5) stated that the call system has been out of order for 14 years, can be activated if a resident requests it, and identified three residents that use manually operated bells.


A review of Resident 1-3's (R1-R3) service plans indicated residents were not evaluated for their ability to use bells as an alternate to the electronic call system.  


On 06/09/2022 these findings were reviewed with and acknowledged by S1 & S3.


Plan of Correction:

Effective immediately the administrator or designee would install new batteries. If that did not work, they would contact with alarm company to ensure proper installation and functionality of each pre-existing alarm in the wing.
