The findings of the kitchen inspection, conducted 10/18/23, are documented in this report. The survey was conducted to determine compliance with the OARs 411-054-0030 for Residential Care and Assisted Living Facilities for Resident Services - Meals and Oregon Health Service Food Sanitation Rules OARs 333-150-0000.
The findings of the re-visit to the kitchen inspection survey of 10/18/23, conducted on 01/05/24, are documented in this report. It was determined the facility was in substantial compliance with the OARs 411-054-0030 for Residential Care and Assisted Living Facilities for Resident Services - Meals and Oregon Health Service Food Sanitation Rules OARs 333-150-0000.
Based on observation and interview, it was determined the facility failed to ensure kitchen practices and protocols were in accordance with the Food Sanitation Rules OARs 333-150-0000. Findings include, but are not limited to:
On 10/18/23 at 11:15 am, the kitchen was observed and the following were identified:
* An open bag of ice in the freezer had a scoop in it;
* No small diameter probe thermometer was available, to more accurately measure food temperatures for thin food items, such as meat cutlets;
* Room trays: beverages and side dishes were uncovered during transport to rooms;
* One garbage can was uncovered, it was located next to cart with room trays which were being prepared for delivery and plastic apron ties fell into the garbage can when staff pulled out aprons to use during meal service;
* Three ceiling lights did not have covers;
* Two ceiling vents in the dishwashing room were dusty; and
* Meal service staff who entered the kitchen to pick up meal plates were not using hair restraints.
The areas of concern were observed by Staff 1 (Head Cook) and the concerns were discussed with Staff 1 and Staff 2 (Executive Director) on 10/18/23. The findings were acknowledged.
1. What actions will be taken to correct the rule violation for each example/resident?The scoop for the ice is no longer stored in the bag. The scoop is stored in a separate container by itself.
A small diameter probe thermometer has been purchased and available for use for thin foods.
Beverages and side dishes are now covered with paper or plastic on room trays.
All garbage cans in the kitchen are now covered and the one garbage can has been moved into the dishwashing area and away from food service area.
All staff members preparing food or assisting with food service will have their hair restrained. Signage has been posted to remind staff.
All cooks and staff members assisting with food service have received individual and group instruction/retraining on the above items. It will be the cooks' responsibility to monitor and ensure compliance on a daily
basis.
The staff members on night shift are responsible for the cleaning of the vents in the kitchen area. They have been provided instruction and the
vents have been added to the deep-cleaning checklist. The lead cook will be responsible for ensuring the cleaning is completed monthly.
Replacement covers have been ordered for the fluorescent ceiling lights. They are due to arrive November 27-29, 2023. It is the maintenance workers responsibility to replace the light covers after replacing the light tubes. The lead cook is responsible for ensuring the covers are in place.
