The findings of the kitchen inspection, conducted 2/13/23, are documented in this report. The survey was conducted to determine compliance with the OARs 411-054-0030 for Residential Care and Assisted Living Facilities for Resident Services- Meals, and Oregon Health Service Food Sanitation Rules OARs 333-150-0000.
The findings of the first revisit to the kitchen inspection of 02/13/23, conducted on 05/09/23, are documented in this report. It was determined the facility was in substantial compliance with the OARs Division 54 for Residential Care and Assisted Living Facilities for for Resident Services Meals, and Oregon Health Service Food Sanitation Rules OARs 333-150-0000.
Based on observation, record review and interview, it was determined the facility failed to maintain the kitchen in good repair and in a sanitary manner in accordance with Food Sanitation Rules, OAR 333-150-000. Findings include, but are not limited to:
Observation of the main kitchen in the Assisted Living on 2/13/23 at 9:45 am revealed the following:
a. An accumulation of food spills, splatters, loose food and trash debris, dirt, dust, black matter and grease was visible on or underneath the following:
* Walls throughout kitchen;
* Floors and drains throughout the kitchen;
* Electrical outlets and light switches;
* Pipes, walls, and flooring underneath sinks;
* Ceiling fire sprinklers;
* Cabinets under the steamtable;
* Dish carts;
* Interior and exterior of microwave;
* Stove and grill, knobs, and doors,
* Thermostat;
* Steam table cabinets;
* Wells inside steam table;
* Toaster and pan under toaster;
* Fryer;
* Dish machine top and underneath;
* Freezer floor; and
* Hood vents.
b. The following areas were in need of repair:
* Bottom of cabinets by steam table had exposed wood corners and edges;
* Stand mixer with flaking/missing paint exposing rusted metal;
* Multiple metal racks throughout kitchen and freezer with rusted areas.
* Prep area tables legs and bottom shelving with visual corrosion and rust making them not cleanable surfaces;
* Hole in the walk under a prep table with a rag stuffed inside the hole; and
* Face of ice machine broken and not securely closing.
c. Kitchen did not have a small diameter thermometer probe for thin foods.
d. Large stand mixer did not have a cover for when not in use.
e. Multiple boxes of food were stored on the freezer floor.
f. Cutting boards were heavily scored and/or stained.
g. Trash cans throughout kitchen did not have lids.
Staff 2 (Dietary Supervisor) was with surveyor during inspection and acknowledged the above findings.
At approximately 12:00 pm, identified areas were reviewed with Staff 1 (Executive director) and she acknowledged the findings.

Wood on cabinets painted. Multiple metal racks & tables replaced. Hole in wall fixed. Ice machine fixed. Cooks have small diameter theoprobes, mixer is covered, have new cutting boards, boxes are off the floor.
Food grade paint received. It was on back order. Mixer will be painted by 3/30/2023. It has to be taken outside to paint and we have had a lot of rain.
