Details

Based on observation, interview, and record review, it was determined the facility failed to ensure food was prepared in accordance with the Food Sanitation Rules OAR 333-150-000. Findings include, but are not limited to:


Observations of the facility kitchen, food storage areas, food preparation, and food service on 07/12/22 revealed:


* Splatters, spills, debris, and drips noted:

-  On counters, storage shelves, cabinets, and in drawers throughout the kitchen;

-  In the refrigerators and freezers;

-  in the interior of the ovens and on the stove tops; and

-  Inside of the microwave.


* The refrigerator in the main kitchen area lacked a thermometer;

* Raw hamburger was stored on a shelf above other foods;and

* Raw bacon was stored in the a refrigerator door shelf with ready to eat food; and

* Undated and unlabeled food items noted in the refrigerators.


* Bagged food items were stored on the floor of the dry storage area.


* The food thermometer lacked a small diameter probe to measure thin foods.


* The floor boards in the kitchen were damaged creating an un-cleanable surface.


* The interior of the cupboard under the sink was damaged and un-cleanable.


* Universal Workers preparing and serving meals were not using aprons.


The Surveyor and Staff 1 (Administrator) toured the areas in need of cleaning and repair. She acknowledged the findings.

Plan of Correction

1) staff has sign off sheet that has been posted in the kitchen there are to sign off on the duties that have been done on their shift, see attached

2) I have purchased new thermometers that will be in the refrigerator

3) The refrierator has been labled where food should go.

4)Have spoke with staff as we had an inservice on dating and labeling food.

5) The bagged food has been picked up and is on a shelf staff had an inservice also

6) Bought a new probe for thin foods

7)Floor boards will be painted as needed

8) under the sink is now a clean washable surface

9) I have bought 12 aprons that will be hanging in the kitchen, and an inservice was done
