The findings of the kitchen inspection, conducted 08/02/22, are documented in this report. The survey was conducted to determine compliance with the OARs 411-054-0030 for Residential Care and Assisted Living Facilities for Resident Services- Meals, and Oregon Health Service Food Sanitation Rules OARs 333-150-0000.
The findings of the first revisit, to the kitchen inspection of 08/02/22, conducted on 11/08/22, are documented in this report. The facility was found to be in substantial compliance with the OARs 411-054-0030 for Residential Care and Assisted Living Facilities for Resident Services- Meals, and Oregon Health Service Food Sanitation Rules OARs 333-150-0000.
Based on observation, interview and record review, it was determined the facility failed to ensure the kitchen was clean in accordance with the Food Sanitation Rules OAR 333-150-000. Findings include, but are not limited to:
The kitchen was toured on 08/02/22 at 10:20 am. The following areas were in need of cleaning:
*Flooring throughout the kitchen had food crumbs and/or debris;
*Flooring inside the walk in refrigerator and freezer had food debris and garbage;
*The industrial can opener had black/brown matter in the holding sleeve;
*The wall behind the warewasher had dried food/spills;
*The shelving under the steam table had black/brown grime;
*Dried matter was found on the slicer, blender and robot coupe;
*The hood was observed with grease and dust; and
*The exit door from the kitchen to the dining room had brown/black matter.
The areas needing cleaning were discussed with Staff 1 (Executive Director), Staff 2 (Chef) and Staff 3 (Director of Sales and Marketing) on 08/02/22. They acknowledged the findings.

*Flooring throughout the kitchen had food crumbs and/or debris;
Flooring throughout the kitchen has been cleaned additionaly a professional cleaning service has been hired to pressure wash and surfaces.
DSD has created a weekly cleaning schedule. DSD has trained staff of proper procedures.
The Executive Director will inspect weekly, and the Regional Director of Operations will conduct monthly audits for 12 months.
*Flooring inside the walk-in refrigerator and freezer had food debris and garbage;
The above task has been completed, and staff will continue to clean daily. DSD has created a weekly cleaning schedule. DSD has trained staff of proper procedures.
Executive Director will inspect weekly, and the Regional Director of Operations will conduct monthly audits for 12 months.
*The industrial can opener had black/brown matter in the holding sleeve;
The above has been completed. A new can opener has been ordered. The task has been added to the daily cleaning schedule. Dining Service Director will complete the weekly inspection for cleanliness. DSD has trained staff of proper procedures.
Executive Director will inspect monthly, and the Regional Director of Operations will conduct spot audits for 12 months.
*The wall behind the dishwasher had dried food/spills;
The above has been completed, and staff will continue to clean weekly Dining Service Director.
DSD has created a weekly cleaning schedule. DSD has trained staff of proper procedures.
Executive Director will inspect weekly, and the Regional Director of Operations will conduct monthly audits for 12 months.
*The shelving under the steam table had black/brown grime;
A professional cleaning service has been hired to pressure wash this area.
To keep the environment free of debris and clean, the Dining Service Director will complete daily inspection rounds. Executive Director will inspect weekly, and the Regional Director of Operations will conduct monthly audits for 12 months.
*Dried matter was found on the slicer, blender, and robot coupe;
The above has been completed, and staff will continue to clean daily Dining Service Directors. DSD has created a daily cleaning schedule. DSD has trained staff of proper procedures. Executive Director will inspect weekly, and the Regional Director of Operations will conduct monthly audits for 12 months.
*The hood was observed with grease and dust;
The hood has been professionally cleaned and is automatically scheduled every six months. The filters are cleaned once a month. The Dining Service Director will complete monthly inspections of filters. Executive Director will conduct monthly audits for 12 months.
*The exit door from the kitchen to the dining room had brown/black matter.
The maintenance director will resurface the door and paint it with appropriate professional paint capable of being cleaned and disinfected daily. Dining Service Director and Executive Director will inspect weekly, and the Regional Director of Operations will conduct monthly audits for 12 months.
