Assisted Living and Residential Care Facilities must operate and provide services in compliance with all applicable State and local laws, regulations and codes.  This report reflects the findings of the complaint investigation conducted 9/27/2022.  The facility was evaluated for compliance with Oregon Administrative Rule 411, Division 54 and if applicable, Oregon Administrative Rule 411, Division 57.  The following deficiencies were identified:

Details

Based on interview and observation it was confirmed the facility failed to exercise reasonable precautions against any condition that may threaten the health, safety, or welfare of the residents. Findings include:


During an onsite interview on 9/27/2022 Staff #3 (S3) stated that all staff prop the laundry room doors open.


Compliance Specialist (CS) observed on 9/27/2022 three separate laundry rooms open and unlocked with both powdered and liquid chemicals open and within reach of anyone who could walk in.


Verbal Plan of Correction: Staff #1 (S1) stated that they will lock the doors of the laundry rooms starting 9/27/2022 and will have an all staff meeting the following week.

Details

Based on interview, observation, and record review it was confirmed that the facility failed to provide three daily, nutritious, palatable meals that are prepare and served. Findings include:


During onsite interview on 9/27/2022 with Staff #2 (S2) s/he stated the food is typically delivered around 10:30am and that the care staff does not put the food into the steam well until closer to 11 and service starts around 11:30


During onsite interview on 9/27/2022 with Staff #3 (S3) s/he stated there was no temperature logs for that day in the book. S3 took scratch paper and started taking temperatures of the food being served for that day ' s lunch. S3 and Compliance Specialist (CS) took the temperatures together showing the BBQ chicken being served was only 126°F and the bratwurst sausage was 130°F. CS asked S3 if s/he was aware of the food danger zone, S3 was unable to provide knowledge of this.


During onsite observation on 9/27/2022 CS observed

·the food on a cart sitting there for over an hour before being served to the residents.

·Food on the cart to not be labeled and dated with no expiration dates.

·Silverware being used to be dirty.

·S3 using a cup of water to  " sanitize "  the thermomotor after each use.

·An unsampled server to not be wearing gloves during the handling of food.  


Record Review on 9/27/2022 of the temperature logbook shows no food temperatures being logged going back to 3 months.


Facility Plan of Correction: In service with staff, retraining staff on how to take temperatures, wear gloves during service of food. Increase meal observation. Will have the assisted living not deliver the food as early.
