Assisted Living and Residential Care Facilities must operate and provide services in compliance with all applicable State and local laws, regulations and codes. This report reflects the findings of the unannounced on-site complaint investigation conducted 09/14/2022. The facility was evaluated for compliance with Oregon Administrative Rule 411, Division 54 and if applicable, Oregon Administrative Rule 411, Division 57. During an unannounced site visit on 09/14/22, Compliance Specialist (CS) reviewed the State Mask Protocol, updated on June 1st, 2020, indicating that masks are required in the facility and showing proper ways to wear masks.
CS observed Staff #2, Staff #3, Staff #4 incorrectly wearing their masks; either not covering their nose and mouth or removing the mask in order to talk to residents and/or other staff members.
Staff #1 indicated that the facility has already implemented a training regarding appropriate mask use but that the facility would be doing so again to ensure staff were properly trained on the importance of wearing masks correctly. S1 also stated that reminders of proper mask wearing would be posted in the upcoming newsletter.
On 09/14/22 the above information was shared with Staff #1.
