The findings of the kitchen inspection, conducted 05/23/23, are documented in this report. The survey was conducted to determine compliance with the OARs 411-054-0030 for Residential Care and Assisted Living Facilities for Resident Services - Meals and Oregon Health Service Food Sanitation Rules OARs 333-150-0000.
The findings of the first revisit to the kitchen inspection of 05/23/23, conducted 08/16/23, are documented in this report. It was determined the facility was in substantial compliance with the OARs 411-054-0030 for Residential Care and Assisted Living Facilities for Resident Services- Meals, and Oregon Health Service Food Sanitation Rules OARs 333-150-0000.
Based on observation and interview, it was determined the facility failed to ensure the kitchen was maintained accordance with the Food Sanitation Rules OAR 333-150-0000.
Findings include, but are not limited to:
On 05/23/23 at 11:10 am, the kitchen was observed and following areas were identified:
* The only handwashing sink did not have soap readily available. It was obtained immediately;
* Dry storage area:
- A bag of flour and sack of onions were stored on the floor;
- Scoops were stored in food bins (granulated sugar, chocolate chips, jasmine rice, flour);
* The facility did not have a small diameter probe thermometer (another thermometer was available and used to take food temperatures);
* The garbage can did not have a lid, the moveable flaps were broken off, exposing the trash to the air;
* The ceiling and wall above the corner cabinet near the stove had a layer of dust, the vent in the same area was dusty;
* The ceiling area around the vent in the center of the kitchen was dusty; and
* The lower corner cabinet near the stove had no attached doors, creating possible contamination of the clean pots and pans.
The areas of concern were observed and discussed with Staff 1 (Cook) and Staff 2 (Administrator) on 05/23/23. The findings were acknowledged.
Kitchen staff were instructed to keep extra hand soap and have readily available when one empties
Flour was placed on a raised shelf
All scoops were removed from the bins
Several new thermometers were purchased
All garbage cans were replaced and have lids on them

Maintence cleaned entire area around cabinets on top and vents 5/25
Cabinet were repaired and reinstalled 5/30
All staff inservice was done on kitchen protocols 6/1/23
Administrator will do weekly inspection of the kitchen
