It was determined the facility was in substantial compliance with the OARs 411-054-0030 for Residential Care and Assisted Living Facilities for Resident Services- Meals, and Oregon Health Service Food Sanitation Rules OARs 333-150-0000.

Details

Based on observation, record review, and interview, it was determined the facility failed to ensure food was prepared in accordance with the Food Sanitation Rules OAR 333-150-000. Findings include, but are not limited to:


Observations of the main facility kitchen, food storage areas, food preparation, and food service on 07/07/22 revealed:


* Splatters, spills, and debris noted:

- Drawer fronts and interiors,

- Cupboard fronts and interiors;

- Stand mixer; and

- Freezer interior;

* Black matter noted on the can opener blade;

* Garbage can by prep table had no lid;

* Dented can noted in dry storage;

* Open bag of rice with scoop lying in the rice;

* Damage to cupboard front;

* No small diameter probe thermometer to measure the temperature of thin foods;

* No documented evidence of monitoring the dish machine temperature or chemical levels;

* Damage to wall in dish machine area;

* Undated and unlabeled food items noted in the reach in refrigerator; and

* Hamburger left to thaw lying on the counter.


The hamburger was still frozen to touch. Staff were directed to thaw in the refrigerator or under cold running water. The hamburger was placed under cold running water to thaw.


Observations of the Memory Care kitchenette and food storage areas on 07/07/22 revealed spills, splatters, and debris in the refrigerator.


The kitchens were toured with Staff 2 (Dietary Manager) and Staff 3 (Maintenance).


The areas in need of cleaning and repair were reviewed with Staff 1 (Regional Director of Operations) and Staff 2. They acknowledged the findings.

- Refrigerator will be cleaned and sanitized

-Weekly cleaning schedule will be implemented

- Garbage can with a lid was purchased

-Director will ensure that all garbage cans purchased have lids

- Dented can was removed and thrown away. Staff re-educated regarding accepting dented cans and disposing of items that are damaged in the sorage areas
