The findings of the first re-visit to the kitchen inspection of 01/18/23, conducted 04/27/23, are documented in this report.  It was determined the facility was in substantial compliance with the OARs 411-054-0030 for Residential Care and Assisted Living Facilities for Resident Services- Meals, and Oregon Health Service Food Sanitation Rules OARs 333-150-0000.

Details

Based on observation and interview, it was determined the facility failed to ensure food was prepared, and the kitchen was maintained, in accordance with the Food Sanitation Rules OAR 333-150-000. Findings include, but are not limited to:


Observations of the Memory Care Community kitchenettes on 01/18/23 revealed:

 

* Spills and debris were noted in the interior of drawers;

* Dish washing racks were stored on the floor;

* Staff were observed to not change gloves between tasks while handling ready to eat foods;

* Caregiving staff, who assisted residents with incontinent care, were not using aprons while serving food; and

* The refrigerators (did not contain protein based foods) were not at 41 degrees Fahrenheit or below.


Staff 2 (Dietary Services Director) and the surveyor toured the kitchenettes. He acknowledged the findings.

Plan of Correction

The facility will ensure compliance by the following:

(1) Cleaning exterior and interior of drawers added to housekeeping task list and NOC care partner task list.

(2) Extra dish washing racks removed and designated space labeled for appropriate area to dry dishes.

(3) Training held on changing gloves in between tasks and signage about proper hand washing provided by 'Ecolab' posted in the kitchen.

(4) Aprons ordered and implemented for use by caregivers who are assisting residents during personal care and then while serving food.

(5) Refrigerator temperature adjusted, and parameters added to ECP to notify Administrator when it is above 41 degrees Fahrenheit or above. Administrator will adjust as needed.
