The findings of the kitchen inspection, conducted 06/14/23, are documented in this report. The survey was conducted to determine compliance with the OARs 411-054-0030 for Residential Care and Assisted Living Facilities for Resident Services - Meals and Oregon Health Service Food Sanitation Rules OARs 333-150-0000.
The findings of the first re-visit to the kitchen re-licensure survey of 06/14/23, conducted 08/24/23, are documented in this report. It was determined the facility was in substantial compliance with the OARs 411-054-0030 for Residential Care and Assisted Living Facilities for Resident Services- Meals, and Oregon Health Service Food Sanitation Rules OARs 333-150-0000.
Based on observation and interview, it was determined the facility failed to ensure kitchen practices and protocols were in accordance with the Food Sanitation Rules 333-150-0000. Finding include, but are not limited to:
On 06/14/23 at 11:10 am, the facility kitchen was observed with the following areas of concern:
* The walk in refrigerator had an uncovered pan containing Jello cake;
* The reach in refrigerator had three uncovered trays of individual plates of sliced pie;
* Lower shelves throughout the prep area of the kitchen had food debris/drips/splatters;
* The vents and interior of hood above stove/grill/convection oven were greasy/dusty;
* The exterior of microwave and its handle had food spills/drips/splatters;
* The ceiling light above coffee/juice station was uncovered;
* Staff not washing hands between glove changes, wearing gloves and touching multiple surfaces without washing hands and/or changing gloves;
* Kitchen staff without hair restraints;
* Non-kitchen staff entered the kitchen to access the beverage station, personally retrieved food items to use for activity events by entering the walk in refrigerator and dry food storage area, including one staff with long unrestrained hair.
The areas of concern were discussed with Staff 2 (Executive Director) and Staff 1 (Diet Aide/Dishwasher) on 06/14/23. The findings were acknowledged.
1) The walk in refrigerator had an uncovered pan containing Jello cake --- Reviewed with DSM and servers that moving forward all open containers in the walk in refrigerator are covered. Corrected immediately. DSM will monitor walk in daily, when DSM is not in the building cook will monitor.
2) The reach in refrigerator had three uncovered trays of individual plates of sliced pie ---Reviewed with DSM and servers that all food in the reach in refrigerator must be covered. Corrected immediately. DSM will monitor reach in daily, when DSM is not in the building cook will monitor.
3) Lower shelves throughout the prep area of the kitchen had food debris/drips/splatters --- Reviewed cleaning task list with cooks and servers. Corrected immediately. Completing daily cleaning task list. DSM will monitor daily, when DSM is not in the building cook will monitor.
4) The vents and interior of hood above stove/grill/convection oven were greasy/dusty --- Reviewed with DSM and corrected immediately. DSM and cooks will be responsible of cleaning vents and hood. Completing daily cleaning task list. DSM will monitor daily, when DSM is not in the building cook will monitor.
5) The exterior of microwave and its handle had food spills/drips/splatters --- Reviewed with DSM, servers and cooks the cleaning task list. Corrected immediately. Completing daily cleaning task list. DSM will monitor daily, when DSM is not in the building cook will monitor.
6) The ceiling light above coffee/juice station was uncovered --- Reviewed with DSM, servers and cooks to notify maintenance. Reviewed with maintenance to complete walk through of kitchen weekly. Servers will monitor daily and notify maintenance.
7) Staff not washing hands between glove changes, wearing gloves and touching multiple surfaces without washing hands and/or changing glove --- Reviewed with servers proper hand washing and when to wear and change gloves. Corrected immediately. DSM will monitor daily, when DSM is not in the building cook will address.
8) Kitchen staff without hair restraints --- Reviewed with servers, hair is to be pulled back during shift and meal service. Corrected immediately. DSM will monitor daily, when DSM is not in the building cook will address.
9) Non-kitchen staff entered the kitchen to access the beverage station, personally retrieved food items to use for activity events by entering the walk in refrigerator and dry food storage area, including one staff with long unrestrained hair --- Reviewed with activities director when needing supplies from the kitchen to ask DSM, servers or cooks. If they are not available to pull hair back and wash hands. Corrected immediately. DSM will monitor daily, when DSM is not in the building cook will address.
