The facility was found in substantial compliance with the OARs 411-054-0030 for Residential Care and Assisted Living Facilities for Resident Services- Meals, and Oregon Health Service Food Sanitation Rules OARs 333-150-0000.

Details

Based on observation, record review and interview, it was determined the facility failed to ensure the kitchen was maintained in accordance with the Food Sanitation Rules OAR 333-150-000. Findings include, but are not limited to:


Observations of the main facility kitchen, food storage areas, food preparation, and food service on 3/9/23 revealed splatters, spills, drips and debris noted on:


- Exteriors and Interiors of cupboards and drawers throughout kitchen;

- Fan blades and cage facing prep area in kitchen;

- Pendent light fixtures above counter in kitchen/sinks were dusty;

- Reach in refrigerators and freezers in kitchen (inside drawers and shelves);

- Interior and exterior of microwave;

- Interior of ice maker and water/ice dispensers; and

- Faucet base of sink on the prep island.

 

* Cutting boards with heaving scoring and staining.


* Lower cupboard where sheet pans stored was dirty and damaged making it not a smooth/cleanable surface.


* Dish machine was not reaching required sanitizing rinse temperature of 180 degrees. Dish machine was not completing a rinse cycle. Facility was not monitoring sanitizing temperature regularly to ensure was functioning properly to ensure dishes were being effectively sanitized.


* Facility did not have a thin diameter thermometer probe as required.


* Counter top mixer was not covered when not in use.


* Multiple cooking pans were not in good repair with the non stick surfaces pealing/scraped off.


* Bulk food bins (such as flour, sugar, etc) observed with scoops stored inside of bins causing potential cross contamination of food items.


* Staff were observed to not change gloves between tasks and handling ready to eat (RTE)foods as well as handling RTE foods (cookies) with bare hands.


Staff 3 (Chef and Person In Charge for kitchen) and the Surveyor toured the kitchen. Staff 3 acknowledged the above findings.


The areas in need of cleaning and repair were reviewed with Staff 1 (Administrator). She acknowledged the findings.

*Audits for kitchen cleanliness and food storage will be done weekly.

*Kitchen equipements such as cutting boards, pots, and pans etc. will be audited quartely to ensure no score marks, stains, or flaws.

*Dishwasher Temps are to be monitored, recorded, and initialed once daily

*Staff to be provided quartely training on proper glove training while in the kitchen and handeling food

*PIC to perform kitchen equipment audits are completed quaretly and report to Administrator any new equipment needs to be purchased.

*Housekeeper to perform all kitchen cleaning audits once weekly, concerns to be reported to Administrator.

*Administrator to monitor dishwashing temp log is being completed on a weekly basis.

*Administrator to provide quaretly training and document regarding when and how to change gloves during meal prep, service, and handeling of any food.

Observations of the main facility kitchen on 05/04/23 revealed splatters, spills, drips, debris and dust on:


* Interior and exterior of cabinets and drawers throughout the kitchen;

* Pendant light fixtures above handwashing sinks;

* Reach in refrigerators and freezers in kitchen (inside drawers, shelves, interior walls and edges of doors);

* Exterior ice dispenser of both refrigerators;

* Interior and exterior of the microwave;

* Hood above the stove;

* Floor drain in cabinet of the food prep island; and

* Exterior surfaces of the warewasher.


The kitchen was toured and the areas in need of cleaning were reviewed with Staff 3 (Chef and Person in Charge for the kitchen) and Staff 4 (Resident Care Manager) on 05/04/23 at 1:40 pm. They acknowledged the findings.

* Daily cleaning check off list for countertops, cabinets, and microwave.

* Pendant light fixtures above sink to be added to weekly housekeeping check off list.

* Reach in refrigerators and freezers in kitchen to be cleaned weekly, this includes ice dispenser.

*Will continue quaretly Hood cleaning through ACME, as well as monthly and as needed to be performed by housekeeping.

*Exterior surfaces of the warewasher to be cleaned at least once daily
