OAR 411-054-0030 (1)(a) Resident Services Meals, Food Sanitation Rule (1) The residential care or assisted living facility must provide a minimum scope of services as follows: (a) Three daily nutritious, palatable meals with snacks available seven days a week, in accordance with the recommended dietary allowances found in the United States Department of Agriculture (USDA) guidelines, including seasonal fresh fruit and fresh vegetables; (A) Modified special diets that are appropriate to residents' needs and choices. The facility must encourage residents' involvement in developing menus. (B) Menus must be prepared at least one week in advance, and must be made available to all residents. Meal substitutions must be of similar nutritional value if a resident refuses a food that is served. Residents must be informed in advance of menu changes. (C) Food must be prepared and served in accordance with OAR 333-150-0000 (Food Sanitation Rules). This Rule is not met as evidenced by: Based on observation and interview, it was determined the facility failed to ensure kitchen practices and protocols were in accordance with the Food Sanitation Rules OARs 333-150-0000. Findings include, but are not limited to: On 12/04/24 at 10:50 am, the facility kitchen was observed to need cleaning in the following areas: *Large food storage bin lids stored under prep counter – food debris buildup; * Sides of grill and deep fat fryer – drips/spills of grease; * Front, sides and doors of convection and regular ovens – food drips/spills; *Shelves below grill and steam table – food debris/spills; * Vent above dishwasher – build up of black matter and/or dust; and *Wall and caulking behind spray hose in dishwashing area – buildup of black matter. The areas of concern were observed and discussed with Staff 1 (Executive Chef) and discussed with Staff 2 (Administrator) on 12/04/24. The findings were acknowledged.

This Plan of Correction is submitted under State regulations and status applicable to assisted living providers. This Plan of Correction does not constitute an admission of liability on the part of Somerset Assisted Living and such liability is hereby denied. The submission of this plan does not constitute agreement by Somerset Assisted Living that the surveyor’s findings or conclusions are accurate, that the findings constitute a deficiency, or that the deficiencies are cited correctly. Please accept this plan as our credible allegation of compliance. C240 1. A. Kitchen, Cooks, Chef and management staff, will be retrained to clean Large food storage bin lids stored under prep counter from food debris buildup. This will be cleaned after meal prep. • Kitchen, Cooks, Chef and management staff, will be retrained to clean after every use the sides of grill and deep fat fryer from drips/spills of grease. • Kitchen, Cooks, Chef and management staff, will be retrained to clean the front, sides and doors of the convection and regular ovens after every use from food drips/spills;. • Kitchen, Cooks, Chef and management staff will be retrained to clean the shelves below grill and steam table from food debris/spills. This will be cleaned after meal service. • Kitchen, and management staff will be retrained to wipe doen the Vent above dishwasher from buildup of black matter and/or dust; and clean the Wall and caulking behind spray hose in dishwashing area from buildup of black matter. This will be cleaned after every shift. 2. • The Administrator or designee will ensure continued compliance through regular review of kitchen cleanliness. 3. • The Administrator or designee will ensure other management employees understand the kitchens cleanliness expectations. 4. • The Administrator or a designee will check for cleanliness as part of daily meal service. 5. • Staff are inserviced monthly. The Administrator or a designee will retrain kitchen staff regarding cleanliness when issues are identified. 6. • The Administrator a designee will be responsible to see that the corrections are completed and monitored. 7.
