OAR 411-054-0030 (1)(a) Resident Services Meals, Food Sanitation Rule (1) The residential care or assisted living facility must provide a minimum scope of services as follows: (a) Three daily nutritious, palatable meals with snacks available seven days a week, in accordance with the recommended dietary allowances found in the United States Department of Agriculture (USDA) guidelines, including seasonal fresh fruit and fresh vegetables; (A) Modified special diets that are appropriate to residents' needs and choices. The facility must encourage residents' involvement in developing menus. (B) Menus must be prepared at least one week in advance, and must be made available to all residents. Meal substitutions must be of similar nutritional value if a resident refuses a food that is served. Residents must be informed in advance of menu changes. (C) Food must be prepared and served in accordance with OAR 333-150-0000 (Food Sanitation Rules). This Rule is not met as evidenced by: Based on observation and interview, it was determined the facility failed to ensure the kitchen was maintained in accordance with the Food Sanitation Rules OAR 333-150-000. Findings include, but are not limited to: Observations of the facility kitchen, food storage, and food preparation areas on 12/05/24 between 1:47 pm and 3:18 pm noted the following in need of cleaning or repair: a. Main Kitchen Area * Exit and entrance doors throughout the kitchen had black and brown scuff marks and were observed to have chipped paint; * Cutting boards were observed to have score marks and gouges deeming them to be uncleanable; * There was a wooden board connected to the steam table that was an uncleanable surface; * Black matter was observed throughout the baseboards of the kitchen as well as where the door jambs connected with the floor; * Walls throughout the kitchen, which included behind the oven, had drips, splatters, and small holes observed in the walls and tiles; * Two out of three garbage cans did not have lids; * Light fixtures were cracked, scratched, and had food debris observed on them; * The light above the oven was not working; * There was no knob on the far right on the stove; * All floor drains were either in disrepair or had dark brown/black debris observed around them; * There were no temperature gauges located in any of the reach in refrigerators observed; * There was a broken ice machine stored under the juice station; * The inside of the microwave had food splatters and debris throughout; * Dust was observed in multiple areas on the ceiling; and * There was a thick layer of dust located where the pans hung on top of the steam table. b. Dry Food Storage Area * The flooring under the stainless-steel wire storage shelves was observed to have built-up black matter under the casings and debris under the shelves; * The door and door jamb had black and brown matter observed as well as gouges and chipped paint; * The wall to the left of the dry storage area was observed to have drips and splatters; * There was no cover on the overhead light; * A ceiling vent was not secured to the ceiling; * Fruit flies were observed around the bananas; and * The flour bin was not completely covered and there was flour debris on the top of the lid as well as the plastic drawers to the right of the bin. c. Stock Room * The reach in freezer unit had food debris located in both the upper and lower areas where food was stored; * The reach in freezer unit had brown debris around the upper and lower doors and door handles; * The door and door jamb had black and brown scuff marks and were observed to have chipped paint and gouges in the wood; * The reach in refrigerator and freezer unit had brown and orange debris in the seal of both doors; and * The two standing freezer units had thick layers of ice observed. d. Warewashing Area * Flooring underneath the warewashing machine had areas that were missing; * The wall behind the sinks had missing caulking; * There were small holes in the tiles on the wall behind the sinks; * The outside of the grease trap had brown debris observed on the top and sides; and * The temperature gauge on the warewashing machine was not working. e. Dining Room * Exit and entrance doors had black and brown scuff marks and/or were observed to have chipped paint; * The wood cabinets under the sink and coffee pot were scuffed and had exposed wood observed, deeming them uncleanable; * The laminate strip above the wood cabinets was not flush with the countertop; and * To the right and behind the sink, there was missing paint, chipped paint, and a gouge in the wall observed. The areas in need of cleaning and repair were reviewed with Staff 1 (Dining Services Manager) and Staff 2 (Administrator) on 12/05/24. They acknowledged the findings.

Main Kitchen Area * Exit and entrance doors throughout the kitchen had black and brown scuff marks and were observed to have chipped paint ** Maintence will repair and paint the doors, ESD will monitor and maintain the scuffs and paint chips monthly as needed. *Cutting boards were observed to have score marks and gouges deeming them uncleanable ** A resurfacing kit was puchased and the Dietary manager will monitor the boards once a week and resurface weekly or as needed. Additionally, new cutting boards are being purchased. * There was a wooden board connected to the steam table that was an uncleanable surface. ** The wooden board and brackets have been removed from the steam table. *Black matter was observed throughout the baseboards of the kitchen as well as where the door jams connected with the floor **Baseboards will be cleaned and/or replaced the will be added to the weekly cleaning task sheet to better reflect the task. *Walls throughout the kitchen which included behind the oven had drips, spltter and small holes in the walls and tile ** The holes will be filled and the walls will be cleaned and/or repainted the walls will be added to the weekly cleaning list to better reflect the task. *Two of the three garbage cans did not have lids ** New trash cans with lids have been ordered. *Light fixtures were cracked, scratched, and had food debris observed on them ** Bids have been obtained and the lights in the kitchen will be repaired and/or replaced and cleaned regularly. *The light above the oven was not working **Bids have been obtained and the lights in the kitchen will be repaired and/or replaced. *There was no knob on the far right on the stove ** Part was on order and has come in since the visit and has since been replaced on the stove. *All floor drains were either in disrepair or had dark brown/back debris around them **The floor drains will be cleaned, repaired or replaced.They will be added to the weekly task list for inspection and cleaning. *There were not tempature gauges located in any of the reach in refrigerator observed **Tempature gauges have been replaced and Kitchen manager will ensure weekly gauges are still in place. *There was a broken ice machine stored under the juice station **The ice machine has been removed. *Dust was observed in multiple areas of the ceiling and there was a thick layer of dust located on the top of the steam table. **The ceiling will be added to the monthly cleaning task sheet and the steam table weekly. B. Dry storage food Area * The flooring under the stainless steel wire storage shelves was observed to have built up black matter under the casings and debris under the shelves ** The dry storage area will be added to the daily cleaning list and a deep clean will be done to bring into immediate compliance. *The door jams had black and brown matter observed as well as gouges and chipped paint. **Chips, scrapes will either be cleaned, painted and/or repaired, and maintained by ESD monthly as needed. *The wall to the left of the dry storage areas was observed to have drips and splatters **Walls will be painted and/or cleaned this will be added to the kitchens weekly task sheet to maintain cleanliness. *There was no cover on the overhead light ** ESD will replace cover and/or light. *A ceiling vent was not secured to the ceiling ** The ESD has secured the vent to the ceiling *Fruit flies were onserved around the bananas **Food safe fruit fly traps were purchased and put in the dry storage area they will be checked weekly. *The flour bin was not completely covered and there was flour debris on the top of the lid as well as the plastic drawers to the right of the bin ** The bin was closed and the flour was cleaned up and has been added to the daily task sheets. C. Stock room *The reach in freezer unit had food debris located in both the upper and lower areas where food was stored ** This was cleaned and will be added to the daily task list *The reach in freezer units had brown debris around the upper and lower area doors and door handles **This was cleaned and added to the daily task list *The door and door jams had black and brown scuff marks and were observed to hve chipped paint and gouges in the wood ** Maintence will repair and/or paint the doors, ESD will monitor and maintain the scuffs and paint chips monthly as needed *The reach in refrigerator and freezer unit had brown and orange debris in the seal of both doors **This unit will be disposed of. *The two standing freezer units had a thick layer of ice observed. **Units will be defrosted and added to the monthly task sheets. D. Warewashing area *Flooring underneath the warewashing machine had areas that were missing **This area of the kitchen floor will be repaired and/or replaced *The wall behind the sink had missing caulking **This area will be recaulked *There is small holes in the tiles in the wall behind the sink **The holes will be filled and repaired *The outside of the greese trap had brown debris observed on the top and sides **The are will be cleaned and added to the weekly task sheets *Tempature gauge on the warewashing machine was not working **Will work with EcoLab vendor to get the gauge repaired and/or replaced. E. Dining Room *Exit and entrance doors had brown and black scuff marks and/or were obseved to have chipped paint ** Maintence will repair and paint the doors, ESD will monitor and maintain the scuffs and paint chips monthly as needed *The wood cabinets under the sink and coffee pot were scuffed and had exposed wood observed deeming them uncleanable **The wood cabinets will be repaired and/or replaced *The laminated strip above the wood cabinets was not flush with the countertop ** Countertop will be repaired and/or replaced *To the right behind the sink there was misssing paint chipped paint and a gouge in the wall observed **This area will be repaired and repainted
