OAR 411-054-0030 (1)(a) Resident Services Meals, Food Sanitation Rule (1) The residential care or assisted living facility must provide a minimum scope of services as follows: (a) Three daily nutritious, palatable meals with snacks available seven days a week, in accordance with the recommended dietary allowances found in the United States Department of Agriculture (USDA) guidelines, including seasonal fresh fruit and fresh vegetables; (A) Modified special diets that are appropriate to residents' needs and choices. The facility must encourage residents' involvement in developing menus. (B) Menus must be prepared at least one week in advance, and must be made available to all residents. Meal substitutions must be of similar nutritional value if a resident refuses a food that is served. Residents must be informed in advance of menu changes. (C) Food must be prepared and served in accordance with OAR 333-150-0000 (Food Sanitation Rules). This Rule is not met as evidenced by: Donham Place (50M048) – Annual Kitchen Inspection C240: Food Sanitation Rule WHO CITATION TEXT Eunsuk Based on observation and interview, it was determined the facility failed to maintain the kitchen in good repair and in a sanitary manner in accordance with Food Sanitation Rules, OAR 333-150-000. Findings include, but are not limited to: 1. On 09/24/25, from 10:10 am to 11:40 am, the kitchen was observed. a. The following areas needed cleaning: * Walls throughout the kitchen – food spills; and * A window was observed with significant spiderwebs present. b. The following areas needed repair: * The ceiling directly above the black refrigerator and doorway to the dry food/ cookware storage room – exposed wire and no outlet covers; * The entry door and door frames leading from outside into the kitchen – dents and gouges; and * The wall under the two-compartment sink was open, unfinished and exposed pipes and the area underneath. 2. On 09/23/25, from 10:10 am to 11:40 am, the dry food storage area was observed to need cleaning in the following areas: * The wall had peeling paint; * Multiple trays – significant black matter; * A window was observed with significant spiderwebs present; * The lower part of the racks – rust; and * Multiple dead and live flies and insects on the ceiling. 3. On 09/23/25, at 11:00 am, the downstairs food pantry was observed. a. The following areas needed cleaning: * Significant spiderwebs – the corner ceiling area. b. The following areas needed repair: * A wall outlet with no cover- exposed wires. 4. Improper food storage: * Refrigerators – undated containers of food; and * Multiple open bread packs were not dated. 5. Other areas of concern included: * Garbage cans, in main kitchen, were uncovered when not in use; * No test strips were available for the sanitation bucket; and * Staff did not check the food temperature before serving or after cooking the meal. The areas of concern were observed and discussed with Staff 1 (CG) and Staff 2 (CG) on 09/24/25 at 11:50 am. The findings were acknowledged.
