OAR 411-054-0030 (1)(a) Resident Services Meals, Food Sanitation Rule (1) The residential care or assisted living facility must provide a minimum scope of services as follows: (a) Three daily nutritious, palatable meals with snacks available seven days a week, in accordance with the recommended dietary allowances found in the United States Department of Agriculture (USDA) guidelines, including seasonal fresh fruit and fresh vegetables; (A) Modified special diets that are appropriate to residents' needs and choices. The facility must encourage residents' involvement in developing menus. (B) Menus must be prepared at least one week in advance, and must be made available to all residents. Meal substitutions must be of similar nutritional value if a resident refuses a food that is served. Residents must be informed in advance of menu changes. (C) Food must be prepared and served in accordance with OAR 333-150-0000 (Food Sanitation Rules). This Rule is not met as evidenced by: Based on observation and interview, it was determined the facility failed to ensure kitchen practices and protocols were in accordance with the Food Sanitation Rules OARs 333-150-0000. Findings include, but are not limited to: On 11/05/25 at 11:00 am, the facility kitchen was observed to need cleaning in the following areas: * Interior of microwave – butter splatters; * Interior of drawers – debris, food crumbs, hair; and * Cabinet door handles – sticky, splatters. Outdated refrigerated foods: * Cedar kitchen refrigerator and Cedar garage refrigerator – outdated yogurt (10/30/25) and leftover items including crushed pineapple (10/14), chicken stock (10/16) and “batter” (10/14). Improper dry storage: * Spruce garage – boxed items on floor including: tortilla chips, cranberry juice, fresh squash, canned soup and six boxes of disposable food storage containers. Other concern: * Cutting boards – worn and scored surfaces. The areas of concern were observed and discussed with Staff 1 (Lead Chef) and discussed with Staff 2 (Community Manager on 11/05/25. The findings were acknowledged.

Plan of Correction

C240 - OAR 333- 150-0000 Resident Services Meals, Food Sanitation Rule: 1a. Interior microwave - butter splatters. The microwave has been cleaned and butter splatters have been removed. 1b. Interior drawers - debris, food crumbs, and hair. Drawers have been deep cleaned and all debris, food crumbs and hair have been removed. 1c. Cabinet door handles - sticky, splatters. Cabinet door handles have been deep cleaned and sticky splatters have been removed. 1d. Cedar garage refrigerator - outdated yogurt (10/31). Correction was made immediately on 11/5/25, and yogurt was discarded. 1e. Spruce garage - boxed items on the floor including: tortilla chips, cranberry juice, fresh squash, canned soup and six boxes of disposable food storage containers. Corrections were made immediately on 11/5/25, food and food storage boxes were removed off the floor and placed on shelves or crates. 1f. Cutting boards - worn and scored surfaces New cutting boards were purchased and old ones were discarded. 2.) A daily, weekly, & monthly cleaning log with instructions will be followed by staff and will include all deficiencies listed above. 3.) The Community Admin & Dietary manager will review the kitchens, including all deficiencies above, once weekly during walk-throughs of the kitchens. 4.) The Community Admin & Dietary Manager will be responsible to ensure all corrections are made and maintained.
