Details

Based on observation and interview, it was determined the facility failed to ensure the kitchen was clean and in good repair in accordance with the Food Sanitation Rules OAR 333-150-000. Findings include, but are not limited to:


The facility's kitchen was toured on 06/23/23 at 10:15 am.


a. An accumulation of food spills, splatters, loose food debris, grease, dirt, dust and garbage was observed on, in or underneath the following:


* Flooring near walls and under appliances throughout the kitchen;

* Walls throughout the kitchen;

* Ceiling panels throughout the kitchen;

* Venting units throughout the kitchen;

* Floor drains;

* Heated holding/proofing cabinet;

* Microwave;

* Fire extinguisher;

* Drawers throughout kitchen;

* Fans;

* Ventilation hood above cooking surfaces;

* Fryer;

* Stove;

* Shelving under convection oven; and

* Warewasher, including plumbing and mechanics.


b. The following kitchen items required repair or replacement:


* Metal pipe in janitorial closet; and

* Wall corner guards between the entrance to the kitchen and dining room.


c. Multiple garbage cans that contained food residue remained uncovered when not in use.


d. Caregivers serving food were not wearing aprons.


The items that required cleaning or repair were observed, and the importance of covering garbage cans and using aprons was discussed with Staff 1 (Campus Director / PAR Administrator) and Staff 3 (Kitchen Manager) on 06/23/23 at 1:10 pm. They acknowledged the findings.

Plan of Correction

1. Facility acknowledges the kitchen was in need of cleaning and minor repairs. The areas called out during inspection and in the report have been cleaned and updated.

Facility acknowledges that caregivers were not wearing aprons.

Facility acknowledges lack of lids on food contaminated garbage cans.

Facility acknowledges the metal pipe concern, this has been addressed by vendor. The pipe is an unused backflow pipe and will be cut off and capped by plumber as able with goal of completing 8/31/2023.

2. The facility has implemented a cleaning schedule to be monitored by the Kitchen Manager.

Aprons will be provided and available for staff to use while serving, requirement will be presented at the caregiver staff meeting on 7/26/23 and in full implementation by 7/30/23.

Garbage can lids ordered and have been implemented 7/25/23.

3. Documentation of the cleaning will be monitored initally at 1 week intervals for 4 weeks, then maintained via quarterly inspections.

Aprons will be monitored at matching intervals to the cleaning inspection.

4. The Kitchen manager will be responsible for initial documentation as mentioned in #3 and then ongoing for the monitoring of cleaning checklist, and compliance with appropriate attire. Quarterly inspections will be done on a rotation of Campus Director and ALF Administrator.
