Based on interview and observations it was confirmed the facility is not exercising reasonable precautions against any condition that may threaten the health, safety, or welfare of the residents. Findings include:

During tour of facility on 05/24/2022 Compliance specialist observed the following:

* No screening for COVID was done upon entering facility

* staff member observed with mask below the nose and mouth

* multiple staff members not wearing masks in the facility
