The findings of the first revisit to the kitchen inspection of 04/26/23, conducted 07/06/23, are documented in this report. It was determined the facility was in substantial compliance with the OARs 411-054-0030 for Residential Care and Assisted Living Facilities for Resident Services- Meals, and Oregon Health Service Food Sanitation Rules OARs 333-150-0000.

Details

Based on observation and interview, it was determined the facility failed to ensure the kitchen was in good repair and appropriate food preparation methods were in accordance with the Food Sanitization Rules OARs 333-150-0000.


Findings include, but are not limited to:


On 04/26/23 at 10:55 am the following concerns were observed:


* The flooring was cracked/torn and stained;


* A brown stain in front of dishwasher;


* Black matter on the floor under the stove and between the stove and one compartment sink;


* Cabinet doors had peeling finishes; and


* The counter top finish worn next to one compartment sink.


*  Frozen hot dogs were observed in a deep sink. Per the cook, they were thawing for the noon meal. No running water was observed at the time (requested cook to turn on water to facilitate proper thawing).


* Use of wood cutting board (for meat products), immediately taken out of use.


* Only two other white non-wood boards were available for food preparation, no colored boards were used to distinguish specific food preparation use.


The areas of concern were observed and discussed with Staff 1 (Administrator) on 04/26/23. The findings were acknowledged.

In addition to an existing four cycle of menus, an alternative food menu has been established and posted at the kitchen, Daining Area, for residents to know what is available if not satisfied with main dish for that mealtime,


Administrator is responsible for this violations will not happen again.


Weekly monitoring will be done by administrator to evaluated and keeping kitchen Log records.

*Kitchen repairs have been scheduled and inspected by management.

*1. All the issues regarding the cracked, torn and stained parts of the flooring have been acknowledged and will be repaired by replacing the entire flooring in the kitchen.

*2. Cabinet doors will be replaced entirely to resolve peeled finish on current cabinets, along with a new countertop next to the compartment sink.



All repairs will be completed by 06/20/2023.

*3. Kitchen staff have been retrained on Freezing and Food Safety Guidelines that include proper Food Thawing instructions. Instructions have also been posted in English and Spanish for kitchen staff to have at hand for reference.
