It was determined the facility was in substantial compliance with the OARs 411-054-0030 for Residential Care and Assisted Living Facilities for Resident Services- Meals, and Oregon Health Service Food Sanitation Rules OARs 333-150-0000.

Details

Based on observation, interview, and record review it was determined the facility failed to ensure the kitchen was maintained in accordance with the Food Sanitation Rules OAR 333-150-000. Findings include, but are not limited to:


The kitchen was toured with Staff 2 (Dietary Manager) on 12/07/22.


Observations of the facilities kitchen, food storage areas, food preparation and food service revealed:


* Splatters, spills, drips, and debris noted on:


- Surfaces and underneath storage shelves, cabinets, and drawers throughout the kitchen and beverage area in the dining room;

- Storage shelves and metal rack shelving throughout the kitchen;

- Food storage bin handles, lids, and surfaces;

- Interiors of reach in refrigerators and freezers in the kitchen and dining room;

- Interiors of cupboards and drawers throughout the kitchen and dining room;

- Walls throughout the kitchen;

- Ceiling above the food preparation area;

- The dishwashing area walls, floors, drain, and equipment;

- The white caulking around the dishwashing area was black in areas;

- Floor drains throughout the kitchen;

- Both sides of the range, grill, and oven;

- Behind and underneath appliances;

- The stand mixer;

- The blade and casing of the can opener;

- Interior of the microwave;

- The cage and blades of a fan; and

- Carts.


* The facility served undercooked eggs. Pasteurized eggs were not available for soft cooked entrees.


* A cup was left in a bag of sugar.


* The laminate was damaged on the drawers, cupboard shelves, and doors in the kitchen and dining room, creating un-cleanable surfaces.


* The cove base throughout the kitchen was damaged creating areas filled with debris.


* Dish racks were stored directly on the floor.


* The wall in the dry storage room, the passage to the walk in refrigerator, below the pass through and the molding in the dining room, and the door frames were damaged, creating un-cleanable surfaces.


The areas in need of cleaning and repair were reviewed with Staff 1 (Executive Director). She acknowledged the findings.

Plan of Correction

We currently have been replacing the lighting, we have ordered new laminate for the kitchen serving window.

We will have all the splatters, spills, drips and debris cleaned from all the surfaces' and underneath all storage. We will be repainting the kitchen walls throughout the kitchen after they have been cleaned. the ceiling will be cleaned of all splatters and painted.

All molding in the diningroom, covebase in the kitchen and door frames will be fixed and painted.

We will find a new solution for the dish racks to get them off the floor.
