Willamette Springs Memory Care is being placed into the EOS program due to the following: 37 number of substantiated findings of abuse at level 3 harm or greater. 4 number of survey citations at level 3 harm or greater. 3 number of non-abuse licensing violations at level 3 harm or greater. In addition to the violations listed above, when notified, the facility failed to correct concerns in an effective or timely manner.
