- The facility failed to have qualified awake direct care staff, sufficient in number to meet the 24-hour scheduled and unscheduled needs of each resident to include supervision and support in accordance with OAR 411-054-0070(1). Per complainant, resident exited RCF unit into a stairwell and fell down the stairs and sustained injuries on 06/12/26.
