- The facility failed to ensure the implementation of services in accordance with OAR 411-054-0036 (2)(g). Per complainant, resident exited RCF unit into a stairwell and fell down the stairs and sustained injuries on 06/12/26. Resident is independent with ambulation when is not anxious or agitated and independent with transfers.
