- The facility failed to have systems in place to respond to the 24-hour care needs of residents, including a Registered Nurse assessment in accordance with facility policy and resident condition in accordancewith OAR 411-054-0045(1)(f)(A); The resident fell down a flight of stairs and suffered multiple broken ribs and a broken clavicle and the facility failed to assess the resident for a significant change of condition after being discharged from the hospital.
