- Description
- Based on concerns and information from multiple pending Adult Protective Service investigations, the Department has determined the Facility is not in substantial compliance with the Oregon Administrative Rules for Residential Care Facilities and that the Facilitys noncompliance placed residents at harm and risk for harm. The failures are violations of the Oregon Administrative Rules. Based on the facility's substantial noncompliance the Department placed the following conditions against the facility's license; restriction of admissions, RN Consultant, staff and RN training and reporting requirements. On 8/15/17 the Department amended the facility's condition to require the following terms; 1 new admit per 7/days with RN Consultant approval, RN Consultant, RN training and reporting requirements. On 12/21/17 the Department issued a 2nd amended condition with the following terms; 2 new admits per seven days with RN Consultant approval, RN Consultant, RN training and reporting requirements. On 3/28/17 the condition was removed.
