Description
Due to approximately 24 APS complaints from approximately October 25, 2017 to February 15, 2018, there was sufficient evidence identifying significant concerns about the facility operation. Resident's suffered harm and potential for harm. A license condition was effective 1/30/18 mandating terms of: Restriction of Admissions; Registered Nurse (RN) Consultant; Staffing requirements; 30day look back following completion of RN Consultant recommendations; and reporting requirements. The license condition was amended and effective on 2/16/18 mandating terms of: Restriction of Admissions; Registered Nurse (RN) Consultant; Staffing requirements (3 direct care staff on day and swing shifts; 2 direct care staff on night shift; no universal worker duties); 30day look back following completion of RN Consultant recommendations; and reporting requirements, including staffing records.
