- Description
- The facility's failure to comply with DHS' rules has caused or is likely to cause serious injury, serious harm, serious impairment or death to a resident or residents. The Department finds that the residents of the facility are at risk of immediate jeopardy. Due to the facility's substantial noncompliance the Department placed the following conditions against the facility's license; restriction of admissions, training requirements, RN Consultant, Medication/Pharmacy Audit, mandatory RN hours 40 p/week, mandatory activity director hours 40 p/week and reporting requirements. The condition was withdrawn on 6/1/18 following a survey revisit completed on 5/23/18 that indicated that the facility was in substantial compliance.
