Description
Facility's acts or omissions created a situation that necessitates DHS to issue a license condition. That is, given the number of citations found during the survey, the widespread nature of these citations, and that the facilitys failures resulted in harm, DHS determined the imposition of a license condition is warranted. Condition placed on the facility's license requires limited admission of residentes of 1 every 14 days, an admiinstrator consultant with RN credentials, and reporting requirements. Condition removed 9/25/18.
