- Description
- The Department has determined an imposition of a condition is warranted due to the lack of adequate administrator oversight, given the number of citations found during the relicensure survey. The widespread nature of these citations and that the facility'ss failures resulted in a high risk of harm.In addition, DHS finds that the residents of the facility are at risk of immediate jeopardy. That is because the facility's failure to comply with DHS rules has caused or is likely to cause serious injury, serious harm, serious impairment, or death to a resident or residents. DHS further concludes that this finding of immediate jeopardy is likely to present an immediate jeopardy to future residents upon admission. As a result, DHS is authorized to impose a condition restricting admissions to the facility. March 27th, 2018 condition was amended and Restrictin of Admission was removed. RN Consultant, staffing requirements, and reporting requirements still apply.
