- Based on the violations listed in relicensure survey #Z83Q11, DHS concludes that Respondent's acts or omissions create a situation that necessitates DHS to issue a license condition. That is because of the number of citations found during the survey, the widespread nature of these citations, and that the facility's failures resulted in harm, DHS determined that the imposition of a license condition is warranted. On 1/9/19 the Department's survey team conducted revisit #2 (#Z83Q13) and found the facility to be in substantial compliance with the Oregon Administrative Rules. The Department's agency partners had no concerns with lifting the condition. On 1/11/19, the Department lifted all licensing conditions against the facility.
