- 10/07/2019 Removed:On September 18, 2019, relicensure survey revisit #1 (6UTX12) was conducted and determined the facility has made substantial progress toward compliance. On October 7, 2019, a telephone conference was held with the Licensee to discuss entering into an agreement for continued compliance.08/09/2019 7th Amended:On July 29, 2019, the Department received a report from the Consultant outlining continued progress in the building and requested to lower the amount of RN and Administrative Consultant hours and reduce the required RN hours. An additional request was made August 2, 2019, to reduce staffing and Activity Director hours due to the decreased census and acuity in the facility. 07/03/2019 6th Amended: On July 1, 2019, the Department received a report from the Consultant outlining the many changes and continued progress in the building, and a request for reduced staffing due to lowered acuity; postponing the RN training until a new RN has been hired; and allow the community 1 admission per every 14 days to allow for training of the Movein process.05/21/2019 5th Amended:On May 10, 2019, the Department received a request from the Respondent to modify the mandated staffing hours for the Activity Director. The Department received a report from the Consultant May 20, 2019, supporting the Respondents request and advised a descriptive plan was developed for group activities that will be offered to residents in the morning04/19/19 4th Amended: On April 18, 2019, the Department received preliminary information from the CBC Licensing Complaint unit indicating the facility is not in substantial compliance with Oregon Administrative Rules and the terms of the current License Condition placed on the facility. The severity and widespread nature of the violations places residents at serious risk of harm.03/14/2019 3rd Amended: Condition amended to reduce number of RN hours and Consultant hours (RN & Admin) 03/01/2019: 2nd Amended: Condition amended to allow 1 resident admission per every 14 days with prior Consultant approval.1/29/2019 Amended: On January 28, 2019, the Department received a request from Respondent to reduce the staffing requirement on all shifts. The RN Consultant also provided recommendations to reduce staffing and Consultant hours.11/21/2018: Based on the violations listed in relicensure survey #6UTX11 completed on 11/15/18, DHS concludes that Respondent's acts or omissions create a situation that necessitates DHS to issue a license condition. Based on the number of citations found during the survey, the widespread nature of these citations, and that the facility's failures resulted in harm or the potential for harm, DHS determined that the imposition of a license condition is warranted.
