- DHS provided Respondent with a Notice of Impending License Condition on January 7, 2019 due to continued noncompliance on relicensure survey revist #2. An Order Imposing License Condition was issued January 9, 2019. On March 11, 2019, the Department requested a conference call with Respondent to discuss the most recent report received from the Consultant regarding concerns of continued noncompliance in the facility. After the conference call the Department received a written plan from Respondent outlining continued efforts in the facility and progression toward achieving substantial compliance. An Amended License Condition was issued March 13, 2019. On August 13, 2019, relicensure survey revisit #4 was conducted and determined the facility has made substantial progress toward compliance. On August 15, 2019, the facility requested the condition be removed and has agree to enter into a Letter of Agreement for continued compliance.
