Description
Violation 1: The Department received preliminary information from Adult Protective Services (APS) on April 14, 2019, that indicated the facility failed to assess and intervene according to Alleged Victim #1's change of condition and protect him/her from repeated inappropriate sexual contact. Preliminary documentation suggest that facility staff were aware of the sexual contact and failed to report abuse or suspected abuse to APS and Law Enforcement.Violation 2: The facility had five unreported instances of substantiated sexual abuse over the last year.
