- On or about February 4, 2019, the Community Based Care survey team completed an licensure survey (#FYGO11) and determined that the facility was not in substantial compliance with the Oregon Administrative Rules governing residential care facilities. In addition, DHS finds that the residents of the facility are at risk of immediate jeopardy. The findings of the survey are listed in Violations 1 through 24 in the Condition. Terms of the Condition include: 1) Restriction of Admissions; 2) RN Consultant; 3) Abuse reporting training; 4) Staff Training Requirements; 5) Reporting Requirements; and 6) resolves all violations. Amending Condition On or about April 23, 2019, the CBC survey team completed a relicensure survey (#FYG012) and determined that the facility was not back in substantial compliance with OARs governing residential care facilities. The facility was cited with 6 tags. Terms of the Condition include 1) Limited Admissions; 2) Administrative/RN Consultant; and 3) Reporting Requirements. On May 7, 2019, the Condition was amended a second time to remove IJ language as none of the tags are IJ and reduced consultant hours to 2 visits per month for 4 hours each time which equals 8 total hours per month and reporting requirements for all terms listed in Condition "as applicable." On 6/11/19, per consultant recommendation, amended Condition for a third time to remove limited admissions. Terms include: 1) Maintain RN Consultant and 2) Reporting Requirements. On July 15, 2019, the CBC survey team completed relicensure survey (#FYG013) and determined the facility was back in substantial compliance with OARs. Agency partners were contacted re concerns and Condition was lifted.
