- The facility relicensure survey (#QROR11) conducted on December 4, 2019, concluded that the Facility's failure to comply with the Oregon Administrative Rules in the areas of but not limited to: Resident Rights and Protection, Abuse Reporting and Investigation, Resident Services: ADLs, Service Plans, Change of condition and Monitoring, Resident Health Services, RN Delegation and Teaching, Medication Administration, and Environmental concerns which constitutes a threat to the health, safety and welfare of its residents.
