- Based on License Complaint Unit information, the facility lacks monitoring of medication administration systems and overall building cleanliness. The findings demonstrate that the Facility is not in substantial compliance with the Oregon Administrative Rules for Residential Care Facilities and that the Facilitys noncompliance placed residents at harm and risk for harm.
