- The facility failed to carry out medication and treatment orders as prescribed in accordance with OAR 411-054-0055(1)(f) per Complainant that a resident required medication three times a day and staff marked medication was administered when they did not physically have the medication in the facility. Another resident is to receive a 25mg dose in the afternoon and they were given a 75mg dose.
