- The facility failed to provide direct care staff sufficient in number to meet the scheduled and unscheduled needs of the residents in accordance with OAR 411-054-0070(1) per complaint the facility is short staffed and sometimes there are only two caregivers for 80 residents. Showers are not on time, long waits for call lights, and not all residents can be escorted due to staffing issues.
