- The facility failed to keep clean and in good repair all interior and exterior materials and surfaces, and all equipment necessary for the health, safety, and comfort of the resident in accordance with OAR 411-054-0300(4)(i) per complaint that the dishwasher has not been working for over a week, the disposal also broke, and water that stinks is all over the kitchen floor and not cleaned up properly.
