- The facility failed to provide direct care staff sufficient in number to meet the scheduled and unscheduled needs of the residents in accordance with OAR 411-054-0070(1) per complaint that staffing is a constant issue, and they need to have at least three staff members present at all times to meet the needs of multiple two person assist residents, and often there are only 2 or 1 staff on duty, and some call lights went unanswered for over 10 hours.
