- The facility failed to have qualified, awake, direct care staff, sufficient in number to meet the 24-hour scheduled and unscheduled needs of each resident in accordance with OAR 411-054-0070(1) Per a complaint "the resident care coordinator (RCC) was working as both a medication aide and caregiver due to shortage of caregivers, and the licensing practical nurse was pulled to provide care."
