Description: On or about December 5, 2018, Alleged Perpetrator 2 (AP2) used physical force to move Alleged Victim (AV) away from another resident, pushed AV against a wall and then forced AV to physically sit down in a chair. On or about December 8, 2018, AP2 physically restrained and removed AV's clothing causing AV to cry out in fear. AP2's actions are considered physical abuse. The facility failed to protect AV from rough treatment which is a violation of Oregon Administrative Rules.
