Description: On or about February 13, 2018, Resident #1 had an unwitnessed fall in his/her apartment that resulted in a hematoma, bruising to his/her face and eye area, and a head injury requiring transfer to the hospital for stitches/sutures. Resident #1's fall risk assessment at admission indicated he/she was a fall risk and had a history of falls prior to admission, but his/her care plan did not include adequate interventions related to falls. The facility failed to properly care plan related to falls. The failure is a violation of resident rights, is considered neglect of care, and constitutes abuse.
