Based on observation, interview and record review it was determined the facility failed to ensure the resident environment remained free of potential accident hazards related to smoking for 1 of 3 (#19) sampled residents reviewed for accidents. In addition, the facility failed to provide appropriate equipment and training to extinguish a possible fire caused by a smoking accident. Resident 19 experienced multiple burns related to smoking and continued at risk for serious injury or harm due to burns as indicated in the attached pages of the survey. Based on the facilitys Immediate Jeopardy in the area of resident safety, the established substandard quality of care and citations in the areas of Resident behavior and Facility Practice and Quality of Care it was determined the facility was not administered in a safe, effective and efficient manner. As a result, this placed all residents at risk for not receiving the necessary care and services to attain or maintain their highest practicable physical, mental and psychological well being as indicated in the attached pages of the survey.
