Description: Resident 1 was admitted with CHF. Resident assessment indicated assistance with ADLs. The resident care plan noted showers and the January, February and March ADL flow sheets reveal the showers were not always initialed. Based on interview and resident record , it was determined the facility failed to ensure sufficient staffing to meet the minimum C.N.A. to resident ratio as directed in the OAR. Relevant portions of the survey are attached. Enforcement was proposed. Oregon Administrative Rule violations occurred.
