- Alleged Perpetrator 2 (AP2) was in his/her frist day of med-aide training with Witness 2 (W2) in the Memory Care unit. AP2 was previously a med-aide in the Assisted Living Portion of the facility. AP2 gave another resident's medications to Alleged Victim (AV) causing AV to be transported to the hospital for treatment. AP2 reported the pictures of residents on the medication cart were black and white photos and the residents look very much alike. AP2 and the facility failed to provide a safe medication administration system which is a violation of resident rights, is neglect of care and constitutes abuse.
