- Alleged Victim (AV) had a known history of aggressive behaviors. On or about September 22, 2018, AV and Witness 1 (W1) got into an altercation in which both residents hit each other. The facility failed to care plan appropriately for AV's known history which is a violation of resident rights, is considered neglect of care and constitutes abuse.
