Findings
The facility failed to monitor Witness 1 (W1) according to his/her known wandering behavior. The failure resulted in W1 wandering into the Alleged Victim's room and shaking him/her while s/he was in bed. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
