- On or about November 18, 2018, the facility failed to provide a safe environment for Alleged Victim (AV) resulting in AV ingested antiperspirant that should have been locked in AV's bathroom cabinet. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse. Based on the incident date and the date the Department processed this case, facility will not be responsible for a paying a civil penalty.
